The Consumer Directed Personal Assistance Program (CDPAP) is a Medicaid program that allows individuals to hire, train, and supervise a personal assistant (caregiver). The duties of a personal assistant for the CDPAP program can vary depending on the recipient's needs. However, typical duties include:

Individuals who qualify for CDPAP in New York State must be a resident of New York State, be eligible for Medicaid, have a medical diagnosis that requires personal care services, and be able to manage their services or have a designated representative who can make decisions on their behalf.
Individuals who qualify for Medicaid must be assessed by a nurse and determined to be eligible for the program. The nurse evaluates the applicant to determine medical need and number of hours needed for care. The consumer then selects and hires a personal assistant. The personal assistant must undergo a health screening and complete the necessary paperwork.
You can hire a family member, friend, or anyone you trust with your care. The personal assistant you choose must meet the hiring requirements set forth by federal and state laws.
CDPAP is a Medicaid-funded program. Medicaid pays personal assistants.
We adhere to all applicable federal and state guidelines related to personal and healthcare information privacy. This starts once we have a formal services agreement, before a caregiver even comes to your family member's home. The written service agreement stipulates the exact restrictions on using your information, which clarifies that even unauthorized individuals will not be given access to information without a client's- or formal consent. Your information is never sold to a third party under any circumstances. Only the office personnel and caregivers who are directly involved in your services will have access to information related to the care of your loved one.
No. You (or an authorized representative) are only involved and are only invoiced after the completion of services. Usually, this happens weekly. You never have to pay caregivers directly. And you'll never be asked to pay an additional fee to any employee.

Medicare does provide an Eligibility Tool to see if your family qualifies. Still, they typically don't pay for the type of hourly personal care service that Your Home Care Agency provides. Caring Shepherd HHC Agency, LLC can provide documentation to third parties, such as long-term care insurance companies, that help you obtain the best coverage possible.
If the caregiver becomes ill or needs to take days off, a backup Caregiver will be provided. We have office staff on call 24 hours a day, seven days a week, in case of any scheduling emergencies. The Office Manager or Care Coordinator. They will ensure someone will provide care for your loved one.
There will be a Care Binder in the home. This binder includes all Caregiver notes, the Care plan for the client, and the time cards for the caregiver. Every week, each caregiver must submit their time car, a carbon copy of the time car, and the detailed weekly letter accompanying it. All of our caregivers work under the supervision of a Registered Nurse, and the RN will also check in by phone and in Person.
Yes. Every caregiver is a Caring Shepherd HHC Agency, LLC employee. To help ensure the safety and well-being of our clients, we perform a criminal history background clearance and cover each caregiver with general liability and workers' compensation insurance.
