  • 2. Who is eligible for CDPAP?

Individuals who qualify for CDPAP in New York State must be a resident of New York State, be eligible for Medicaid, have a medical diagnosis that requires personal care services, and be able to manage their services or have a designated representative who can make decisions on their behalf.

  • 3. How do I get enrolled in CDPAP?

Individuals who qualify for Medicaid must be assessed by a nurse and determined to be eligible for the program. • How is the privacy of my information maintained?

We adhere to all applicable federal and state guidelines related to personal and healthcare information privacy. The written service agreement stipulates the exact restrictions on using your information, which clarifies that even unauthorized individuals will not be given access to information without a client's- or formal consent. Your information is never sold to a third party under any circumstances. Only the office personnel and caregivers who are directly involved in your services will have access to information related to the care of your loved one.
